Destiny Health Empowerment Members who are
part of the Brown County Healthy Lifestyle CoOp have access to the ChoiceCare
Network. ChoiceCare is one of the largest health care provider networks in the
United States, offering customers access to over 447,000 providers and 2,900
hospitals from coast to coast. The ChoiceCare Network is wholly owned
subsidiary of Humana Inc. (Louisville, KY), one of the nation's largest
publicly traded health benefits companies, with approximately 11 million
medical members.

Cofinity (previously PPOM)
Cofinity, previously named PPOM, is the largest independent PPO network in
the Midwest serving the states of Michigan, Indiana and Ohio as well as parts
of Illinois, Kentucky and Wisconsin. The Cofinity network includes more than
60,000 physicians and other healthcare providers and more than 480 hospitals.

Cofinity (previously Sloans Lake Preferred Health Network)
Cofinity, previously named Sloans Lake Preferred Health Network, is the
largest independent PPO network in the state of Colorado. The Cofinity network
provides its members access to over 98 percent of Colorado hospitals and more
than 15,000 providers in 18,500 service locations throughout the state.

ECOH
Members of The Destiny Health Empowerment Plan have access to the Employers'
Coalition on Health (ECOH), two distinct networks with providers in northern
Illinois and southern Wisconsin: ECOH Network, with more than 1,500 primary and
specialty care providers and 21 hospitals; and ECOH River Valley, with more
than 1,200 primary and specialty care providers and 19 hospitals. ECOH was
founded in 1995 by Rockford, IL-area employers that shared a common goal of
controlling health care costs through direct contracts with providers while
focusing on quality and appropriate delivery of care.

HFN, Inc.
Members of The Destiny Health Plan have access to HFN's Platinum Network,
which includes more than 50 hospitals, 14,000 physicians and 1,880 facilities
throughout Illinois and surrounding states.

HealthEOS (Formerly AHC)
Members of The Destiny Health Plan in Wisconsin have access to HealthEOS, an
independent, statewide PPO network that includes 125 hospitals, 500 ancillary
care facilities, 17,000 practitioner locations and every one of Wisconsin's
major health systems. HealthEOS by MultiPlan represents the consolidation of
highly regarded independent networks Wisconsin Preferred Provider Network
(WPPN), Associates for Healthcare (AHC), Health Care Network (HCN) and
Touchpoint Preferred, offering Wisconsin employers a comprehensive capability
for managing the cost of claims.

HealthLink
Members of The Destiny Health Plan have access to HealthLink, which
contracts discounted health care services with more than 25,000 participating
physicians and other health care providers, and more than 300 hospitals and
facilities. HealthLink's network programs offer flexibility and accountability
to meet a variety of consumer needs.

HealthPlus
HealthPlus offers up to three provider networks available only to Destiny
members affiliated a special arrangement for the Wausau Area Chamber of
Commerce and Portage County Chamber of Commerce and Economic Development
Corporation. The HealthPlus networks offer access to local providers in
the greater Wausau-Stevens Point area. Depending on the HealthPlus
network, the selection of providers may be specific to a particular local
health care delivery system - Aspirus Network or Ministry Health Care.

HealthSmart (PPO and GEPO)
Members of The Destiny Health Plan have access to HealthSmart Preferred
Care, a Preferred Provider Organization (PPO) whose network of health care
providers (doctors, hospitals, etc.) will provide health services for members
at discounted rates.

National Capital PPO (NCPPO)
A regional leader, National Capital PPO (NCPPO) offers access to a broad
network of physicians, hospitals and other health care professionals who have
agreed to provide health care services at negotiated, discounted rates.
NCPPO network is geographically and specialty balanced to ensure convenient
access to contracted health care services.
NCPPO's network includes 100 hospitals and more than 20,000 physicians and
other ancillary professionals throughout Delaware, District of Columbia,
Maryland, Virginia, West Virginia and portions of New Jersey and Pennsylvania.
PPO enrollees have the freedom to choose any physician or health care facility.
Network participating physicians and facilities have agreed to accept
discounted rates of reimbursement for treatment or services.

OneNet PPO offers access to the mid-Atlantic region's largest PPO network
which includes over 57,000 participating physicians and other health care
practitioners, and 350 hospitals.

Preferred One
Members of The Destiny Health Plan in the Minnesota area have access to
Preferred One, a network of more than 12,700 physicians (6,300 primary care and
6,400 specialists), and 240+ hospitals. The online provider directory enables
members to search for individual practitioners, clinics, urgent care facilities
and hospitals by name, location (within a specified mileage distance), county
and city.

Preferred Plan, Inc.
Members of The Destiny Health Empowerment Plan have access to Preferred
Plan, Inc. (PPI), an additional provider network for groups and individuals
outside of the Chicagoland area. PPI currently has more than one
million members. Members enjoy convenient access to more than 300 quality
hospitals and over 32,000 physicians.

Prevea
Members of The Destiny Health Empowerment Plan have access to Prevea Health
Network's Preferred Provider Organization (PPO), which includes an extensive
network of health care professionals located throughout Northeastern Wisconsin
and the Fox Valley. Prevea Health Network's goal is to provide quality health
care for members and their families in an effective and friendly manner, while
delivering cost savings for members and employers. Its website includes a
directory of physicians, clinics, hospitals, and other medical
professionals/facilities that are part of the Prevea Health Network.

Private Healthcare Systems (PHCS)

Inc., an extensive nationwide network of more than 450,000 Medical

Texas True Choice, Inc. (TTC)
Members of the Destiny Health Plan in Texas have access to Texas True
Choice, Inc. (TTC), one of the largest, independent Preferred Provider
Organization (PPO) networks in the state. TTC has more than 36,000
providers and 352 acute care hospitals throughout Texas serving more than
780,000 members.
The TTC website offers its members the option to search by provider name,
specialty, facility, city or zip. Members can nominate a provider to be
contracted with TTC, or print a customized provider directory. Once you
have opened the below link simply select TTC Statewide to locate a
provider.
